4-H Record Sheets

The following 4-H record sheets are currently available for 2008:

The record sheets listed below can be printed off and completed by hand, or you can type right into the pdf file and print out your completed record sheet.  (* denotes forms that are not yet available as "fillable" pdf files)

Unless otherwise noted in the project rules in the Porter County 4-H Handbook, Levels and Divisions are divided as follows:

Level 1:  Grades 3, 4, 5

Level 2:  Grades 6, 7, 8

Level 3:  Grades 9, 10, 11, 12

Level A:  Grades 3, 4

Level B:  Grades 5, 6

Level C:  Grades 7, 8, 9

Level D:  Grades 10, 11, 12

Division I:  Grades 3, 4

Division II:  Grades 5, 6

Division III:  Grades 7, 8

Division IV:  Grades 9, 10

Division V:  Grades 11, 12
