This page contains the necessary forms for 4-H members and parents to complete in order to attend La Porte/Porter County 4-H Camp. Forms are not completed unless all signatures are on each form. 4-H members will be considered signed up for 4-H Camp once their registration form, health form, permission form, behavior expectations form are all returned with payment. 2010 LaPorte/Porter County 4-H Camp will be announced soon.
