Congratulations on your interest in joining the largest youth serving organization in the United States! If you don't know where to start, refer back to the General Information page for information on how to contact one of the outstanding 4-H clubs in Johnson County.
Johnson County 4-H enrollment for 2009 officially opens in January of 2009. We request all enrollments to be received no later than April 1st, 2009.
This enrollment form has interactive form fields! You can fill it out from your computer, but since it requires physical signatures, you must print this form, have it signed and turned in. You can also print without filling it in and complete it by hand.
Enrollment forms will not be accepted at the Purdue Extension - Johnson County Office unless they are COMPLETELY filled out. Please turn completed enrollment forms into your club leader, along with your $10 enrollment fee.
